Workers' Compensation Data Collection Organizations

Use this form to specify the data collection organizations (DCOs) to which you must submit employee injury reports for workers' compensation.

DCOs are organizations that collect data on behalf of their members. Most states have a rating bureau or an insurance association that is considered a DCO for workers' compensation claims. Sometimes a claim report must be sent to multiple DCOs for an interstate injury claim.
